Farukh Chaudhry, CPhT
Managing Partner/Owner
Farukh is a nationally certified pharmacy technician. Committed to helping people in the community and serving patients from all walks of life, Farukh brings a compassionate, results-driven approach to his work. His "can-do" attitude exemplifies his willingness to go above and beyond what is expected, ensuring each patient receives exceptional, personalized care. With over 20 years of experience in healthcare and pharmaceutical services, Farukh specializes in pharmaceutical compounding, patient consultation, and creating effective health protocols that enhance patient outcomes. A lifelong learner, he remains dedicated to integrating the latest advancements in medical science and technology to uphold the highest standards in pharmacy care.
Saumiin Calcuttawala, PharmD, MS
Managing Partner/Pharmacist
Saumiin graduated from Butler University, obtaining his Pharm D and continued his education at Indiana University to receive his Master's of Science in Health Informatics. With his previous experience in chain pharmacies, he has brought great knowledge since joining Dr. Aziz in 2023. Specializing in compounding, retail, professional grade supplements, and specialty pharmacy, Saumiin is passionate about helping patients find therapeutic solutions for their needs and works closely with providers to find unique therapies to help treat their patients. When not at work, Saumiin enjoys watching movies, traveling, and sorting through his sports card collection!
Kurt Moyer, RPh
VP of Clinical Services
Kurt started at Dr. Aziz Pharmacy with an externship through Butler University, starting in 1991, and has been here ever since. With a Bachelor's of Science in Pharmacy, he specializes in hormone replacement therapy, pain management, and nutritional supplement consultations. Kurt is passionate about helping patients find a treatment plan that improves their quality of life. Being a problem solver, he helps patients find the root cause of their problems, rather than giving temporary solutions. In his free time, Kurt can be found in his garden, cooking, or spending time with his family.
Leslie Fahey, RN
Registered Nurse
Leslie graduated from IVY Tech State College in 2001, with her Associate's degree in Nursing. She joined the Dr. Aziz team in 2021, bringing with her a diverse background in various specialties and over 20 years of experience as a Registered Nurse. From assisting in surgeries to working with mental health, her passion for nursing stems from the opportunities it provides and wanting to give her patients the best possible care. When not at the pharmacy, Leslie enjoys staying active with biking, working out, playing soccer and golf, and likes to cook and listen to live music.
Tanya Smith, PA-C
Primary Care Specialist
Tanya graduated with a Bachelor's of Science in Physician Assistant Studies from Butler University College of Pharmacy and Health Sciences in 2001. She later received her MBA in 2011, became a Certified Medical Examiner in 2017, and is currently CPR certified and NCCPA Board certified. Tanya began her journey with Dr. Aziz Pharmacy in 2016, bringing with her over 10 years of experience in primary care. She is passionate about the pharmacy's compounding abilities and its experienced retail pharmacists. When not at the pharmacy, Tanya enjoys reading, partaking in outdoor activities, learning about art, and spending time with her friends and family.
Kat Schwedler
DME Manager & Certified Pedorthist
Kat has over 30 years of experience in pedorthics, orthotics, and prothesis, making her our specialist in diabetic footwear and orthosis. She started at Dr. Aziz Pharmacy in 2019 with the goal to provide patients the care they need while going above and beyond their expectations. In 2024, she received the American Board of Certification in Orthotics and Prosthetics. Kat is passionate about providing diabetic patients with the best possible footwear and care to help them live comfortably. In her free time, she enjoys woodturning and photographing wildlife.
Jeffrey Pacheco
Homecare Rep/DME Billing Manager
Jeff graduated from Purdue School of Engineering Tech at Indianapolis in 1996, with a Bachelor's of Science in Mechanical Engineering Technology. He has over 10 years of drafting experience using CAD software at Verizon FTTP Design Center, bringing with him a great understanding of data collection and processing. He has been with Dr. Aziz Pharmacy since 2012 and specializes in insurance coverage verification and ordering supplements and is passionate about insurance reimbursement. When not at the pharmacy, Jeff enjoys fishing, listening to music and podcasts, playing video games, and reading.
Erin Moffitt
Patient Service Rep
Erin was hired at Dr. Aziz Pharmacy in 2000 with a passion for helping others to her best ability. Her role at the pharmacy includes helping patients and customers with whatever supplies they need, whether that be finding something for them in-store or special ordering items not in stock. Outside of the pharmacy, Erin likes to take her dog on walks, go camping, and watch her son play baseball on his school's team as well as his travel team,
Cyndie Baker
Marketing/Patient Care
Cyndie graduated from college in 1982 with a degree in Marketing. She has previous experience managing the supplement department at a local vitamin and supplement store in Fishers, IN. Her goal when hired at Dr. Aziz Pharmacy in 2020 was to continue to provide exceptional patient care and assist our Primary Care Specialist, Tanya, with whatever task she may need. When not at work, you can find Cyndie spending time with her family and grandkids or finding antiques to add to her collection.
Beth Logsdon
Office Manager
Beth started her career at Dr. Aziz Pharmacy in 1999 with a passion for the patients and the community. She is a team player and ensures the needs of both the employees and the patients are met. She loves that the pharmacy is always there for its patients and the community and is committed to bringing the personal touch that makes this pharmacy unique. Outside of the pharmacy, Beth enjoys vacationing in Florida and loves spending time with her son and his family, especially her granddaughters.
Ashraf Abdelsalan
Laboratory Manager
Ashraf obtained his Bachelor's degree in 1999, from Cairo University in Egypt. He has previous work experience at the well-known pharmaceutical company in Egypt, Borg Pharmaceutical Industries, as a Business Unit Manager in charge of managing a team of over 40 pharmacists. He began his career with Dr. Aziz Pharmacy in 2020 with a passion for providing perfect service to his patients. In his role as Laboratory Manager, he works hard to ensure the compounds created are precise and of high quality. When not at work, Ashraf enjoys playing soccer and spending time with his wife and their three awesome girls.
